As you complete the online Disability Application process, we will ask you to provide only personal information we need to process your application.
Your personal information is safe and secure with us. After you provide a few details about yourself, we will give you a re-entry number. Make sure you print or write this number down. While we recommend you complete and submit your application all at once, we know that this is not always possible. Your re-entry number will allow you to save your application and return at any time. When you return to your saved information, you'll be able to continue right where you left off. If you lose your re-entry number, you will have to start over from the beginning, so please keep your re-entry number in a safe place.
If you're completing this application for yourself, there are four steps for you to complete during the disability application process: Step 1, provide background information. Step 2, provide disability information. Step 3, sign medical release. Step 4, confirmation. If you're completing this application for someone else, the applicant will need to print, sign, and mail the medical release form, also known as the "Authorization to Disclose Medical Information," to Social Security Administration. Receive a copy of the application summary. They will need to sign and return it to their local Social Security office. In step 4, we will give you a confirmation number to check the status of your application. On this page, we will also provide you with the information on what you need to do next.
