What is MAVI?

MAVI is the only independent living center in Puerto Rico headquartered  in San Juan and a satellite office in Ponce (South).  We promote the independent living philosophy through peer counseling, advocacy, information and referral and skills development.  Other services provided include:  adaptive housing, accessible transportation, personal assistance services, psychology, assistive equipment and supported employment.  We promote client control and productivity and their integration and inclusion in all aspects of our society.

What is Mental Health Association?

The Mental Health Association, Cortland County, provides the following: Family Support Services, Reach-Basics bimonthly lectures on or about mental health and three newsletters. We fulfill these activities through the labor and activities of a mental consumer work force. Current employees are responsible for all secretarial work, printing and copying, collating, envelope stuffing, preparations and reports for US Postal Bulk Mailing regulations, cleaning, composing news articles, maintaining work force statistics etc. Each employee is assigned to his/her own work area which includes a desk, chair etc.A newly established work program is our Courier/Messenger service which we plan to enlarge and one day provide this service to local businesses.

What is the Mental Health Association of Erie County, Inc.?

The Mental Health Association of Erie County, Inc. is a private non-profit corporation whose core mission is to improve care and treatment for persons who may have a mental illness and to promote mental health through education, advocacy, public information and prevention activities. Client Advocacy Service is a department of the Mental Health Assn. of Erie Co., Inc. which provides civil legal services at no charge to mental health consumers in Erie and Niagara Counties. Most of the legal advocacy involves public benefits including supplemental security income, social security disability benefits, public assistance, Medicare and Medicaid. CAS also provides training on legal issues to professionals, consumers and family members.

What is the Mental Health Association of Essex County?

The Mental Health Association in Essex County, Inc. is a not-for-profit human services organization chartered to: increase community awareness of mental illness, promote improved functioning for people with mental illness, and provide advocacy, referral, and preventive services.

The programs of the Mental Health Association include:

  • Supported Housing: This program enables people to locate and secure housing in their own communities. Assistance with the costs of renting provided to those who qualify.
  • Assisted Competitive Employment and Supported Employment: MHA staff is in place to help people determine the career, which makes the most sense. We also provide assistance on developing the tools for the work, finding jobs and retaining employment.
  • Intensive Case Management: Each individual in this program receives several home visits every month. This is intended to ease the sometimes-difficult task of community living and to assist with the achievement of success and fulfillment.
  • Community Case Management: Our case managers provide support, education, training and information to assist people who wish to live independently within their communities. The case mangers make visits to people in their own homes.
  • Community Centers:   MHA has sites in local communities where people can go in order to establish friendships and support. The centers are located in Westport, Ticonderoga, Port Henry, Elizabethtown, and Lake Placid.
  • Crisis Alternatives Program: This program is new to MHA in late 1998. Its goal is to provide support and assistance to people who are feeling the most intense anxieties. Staff works in the evenings and on weekends to provide telephone counseling, home visits, and respite apartments to those to those in need.
  • Self Help: MHA serves as a conduit for funding for consumers of mental health services involved in self-advocacy, education, and empowerment.
  • Education Assistance: Many people in our community have not achieved the lower level of education they wish for themselves. This program seeks to reduce barriers for people who agree to actively pursue their GED, Associates, Bachelors, Graduate Degrees, or other academic endeavors.

What is the Peer Advocacy Program?

Peer Advocacy Program, which outreaches to recipients of mental health services and their families, recruits and trains people to participate in the mental health services planning process, identifies educational needs and has established an advocacy informational center and network. What is the Mental Health Association in New York State, Inc.?

The Mental Health Association in Ulster County, Inc., (MHA) was chartered as a not-for-profit organization in 1959 by a group of physicians and concerned individuals to develop a coordinated citizen's movement to promote mental health and to work toward improved care and treatment for persons with mental illnesses.   The mission of the MHA is to create communities that respect diversity and are dedicated to the full participation of all persons.

To achieve its mission, the MHA  sponsors a variety of educational, advocacy and service programs, including mental health education, rehabilitation, residential services, family and youth support services and therapeutic foster care.

The following is a list of our programs offered:

  • Client Services Mental Health- GOAL:  To provide opportunities for persons recovering from mental illnesses and individuals with developmental disabilities to participate in recreational, educational and leisure activities that enhance their socialization skills and community integration.
  • Education and Advocacy Services- GOAL:  To develop educational programs which meet the needs of Ulster County residents through education, training and advocacy services.
  • Residential Services- GOAL:  To provide a variety of community-based housing alternatives to enable persons recovering from mental illnesses to become and to remain a part of the community.
  • Family-Based Treatment Services- GOAL:  To maintain/provide a family environment for children and youth with special emotional needs, who might otherwise be placed in a more restrictive setting.
  • Family and Youth Services- GOAL:  To provide programs that enhance family life by enabling the development of healthy relationships between parents and children and promote the well-being of all children.

What is the Mental Health Resource Center?

The Mental Heath Resource Center provides outpatient treatment for children and adults with mental, emotional and behavioral problems.  Our professional staff provides a variety of therapeutic methods, including: individual, family, group, couple, and play therapies; stress management; follow-up to psychiatric hospitalization; and, medication monitoring and administration.   The Center also provides group therapy for mentally ill chemical abusers (MICA).   As the convener of the Youth Incentive Program for Essex County, the Center plans for youth in danger of residential placement or those youth returning form residential placement to the community.

What is the Metropolitan Hospital Center/ CSS Psychiatric Day Treatment Center?

Located in the East Harlem section of Manhattan, the CSS Psychiatric day Treatment Program at Metropolitan Hospital center provides full-time continuing drug treatment for severely and persistently psychiatric clients.  We serve sixty clients and have a multidisciplinary staff composed of a psychiatrist, psychologist, nurse clinician, two social workers, two rehabilitation counselors, two activity therapists and one recreational therapist.  All staff are professionals.   We have art therapy rehabilitation students and peer specialist interns who work part-time at internships at our program.  We have a large Hispanic population and have four bilingual staff members to serve them.  We provide case management, psychopharmacology, health screening, assistance with benefits and housing applications, individual and group counseling, supportive and cognitive therapy, educational vocational rehabilitation assessments, counseling and referrals.  There is no time limit.   Clients are expected to attend Monday through Friday from 9:00am to 3:00pm.   The program offers a milieu where clients have daily structure, support and opportunities to socialize and recover.  Many of our clients are referred from inpatient psychiatric hospital units and outpatient psychiatric clinics and housing programs.  While at CSS clients have opportunities to volunteer to work in the program running self-help groups and be involved in counselor-run businesses and vocational activities.

What is MAVI:  Caribbean Benefit Planning Project?

Movimieto para el Alcance de Vida Independiate or MAVI is the only Center for Independent Living in Puerto Rico.  We are a not-for-profit organization serving people with disabilities incorporated under the Commonwealth of Puerto Rico.  Our mission is to promote the independent living philosophy and consumer control in all aspects of his/her life.  We advocate for the inclusion, integration and productivity of people with disabilities in all aspects of society.

We partner with the Virgin Island Association for Independent Living to create the Caribbean Benefit Planning Project to serve 20,000 beneficiaries with disabilities in this area.  The project will provide the benefit planning, assistance and outreach in this region through the creation of a 1-800 line, a webpage, development of educational and training opportunities and networking.  There will be case management services to those individuals who desire to develop an individual work plan.

Among other limited services that we provide include:  personal assistance services, adaptive housing, psychology and transition.  We have a main office in San Juan and a satellite office in Ponce.

What is the Muscular Dystrophy Association?

Since its founding in 1950, the Muscular Dystrophy Association has become on of the nation's largest and best known voluntary health agencies. MDA provides comprehensive medical services to tens of thousands of people with neuromuscular diseases at more than 200 hospital-affiliated clinics across the country. The Association's worldwide research program, which funds some 400 individual scientific investigations annually, represents the largest single effort to advance knowledge of neuromuscular diseases and to find cures and treatments for them. At facilities in MDA's nationwide network of hospital-affiliated clinics, patients receive diagnosis and follow up care from specialists in neuromuscular disease. What NAMI - Familya?

NAMI - FAMILYA of Rockland County serves families of people with mental illness through support, education, and advocacy. We provide a wide variety of support services, informational programs and materials for family members, mental health professionals, and consumers of mental health services, as well as the general public.

Founded in 1981 by a small group of families with relatives with mental illness, the organization has grown steadily in number and influence. NAMI - FAMILYA is the Rockland County affiliate of NAMI-NYS and NAMI (the National Alliance for the Mentally Ill.) In concert with thousands of similar grassroots organizations throughout NYS and the U.S., we are recognized as a significant advocate in defending the rights and advancing the interests of individuals with mental illness.

The organization is committed to improving the lives of people afflicted with mental illness, offering hope and assistance to their families, advancing the search for cures, fostering understanding and combating discrimination and stigma through public education. Dedicated volunteers carry out our activities and programs.

Regular meetings are held on the first and third Wednesday of each month in room 119, Building F of the Dr. Robert L. Yeager Health Center, Sanatorium Road, Pomona, New York. Our RAP sessions provide emotional support to family members, as well as information and guidance regarding coping techniques and available resources. Our educational programs on the third Wednesday of each month feature experts on a variety of relevant topics dealing with such issues as research, treatment and rehabilitation, services and entitlements, etc. Our monthly newsletters are as on-going source of information and education. We periodically offer 10-week Family-to-Family Supportive Training course to interested family members at no charge. This series is invaluable to people whose loved ones have recently been diagnosed with mental illness.

We also engage in many educational outreach projects to the community at large. These include annual presentations in health, sociology, and psychology classes at several local high schools colleges, public forums on a variety of topics imparting on people with mental illness, and dissemination of brochures and information about mental illness. We provide in-service sensitivity training, both for Rockland County Department of Mental Health and the Rockland Psychiatric Center.

What is the National Alliance for the Mentally ILL of New York City?

The National Alliance for the Mentally ILL of New York City, Inc. (NAMI-NYC Metro) is dedicated to improving the lives of people with neurobiological disorders (mental illness) and their families and friends through support, education, advocacy, and research. Founded in 1979, NAMI-NYC Metro is a grassroots, non-profit, self-help, organization of family members and friends of the mentally ill, as well as people who suffer from mental illness themselves ("consumers"). NAMI-NYC Metro is one of the largest chapters of the National Alliance for the Mentally ILL (NAMI) and the New York Alliance for the Mentally ILL (NAMI-NYC)

NAMI-NYC Metro offers practical support, useful education, comfort and understanding to those in the greater New York City area who suffer or have family members suffering from neurological disorders: primarily schizophrenia; bipolar disorder (manic-depression); depression; panic and anxiety disorders, including obsessive-compulsive disorders; multiple diagnosis, including mentally ill, chemically addicted (MICA) consumers; and borderline personality disorders.

What is the National Association for Visually Handicapped?

NAVH is the only voluntary health agency which serves the "hard of seeing"  exclusively--not totally blind, for whom over 800 helping agencies exact. Our services include visual aids counseling and distribution, large informational materials and a free-by-mail loan library of over 7,000 titles, educational outreach to lay persons and professionals, emotional support on individual and group levels, advocacy, and referral services. What is the National Down Syndrome Society?

The National Down Syndrome Society was established in 1979 to ensure that all people with Down syndrome have the opportunity to achieve their full potential in community life. What is National Puerto Rican Forum, Inc.?

The National Puerto Rican Forum, Inc. (NPRF) is a 43 year old non-profit organization seeking to improve the socio-economic conditions of Latinos/as and other underserved communities through information, advocacy and direct services. Founded in 1957, NPRF is the oldest national Puerto Rican organization in the United States. Initially incorporated as the Puerto Rican Leadership Forum, the organization brought together leading Puerto Ricans under the guidance of Dr. Antonia Pantoja and other influential Puerto Ricans to conceive ways of ensuring economic self-sufficiency and effective citizenship to the new wave of Puerto Ricans immigrating to the United States. The vast majority of these immigrants found their way to New York City. The "Leadership Forum" was to be the place to create and engineer other organizations to carry out activities for social change. Thus ASPIRA, Boricua College and the Puerto Rican Association for Community Affairs were all founded through NPRF. Currently the organization has a physical presence in the Bronx and Manhattan, New York as well as Chicago, Illinois. NPRF runs successful vocational and Adult Basic Education programs as well as preemployment and job placement programs. NPRF has helped over 100,000 individuals attain self-sufficiency through programs in pre-employment training, Vocational ESL and GED preparation. NPRF also hosts symposia on pertinent issues within the world of workforce development. Currently the organization is looking to further the scope of its activities by offering technical/capacity building assistance to CBO's.

What is New Alternatives for Children, Inc.?

Founded in 1982, New Alternatives for Children (NAC) is the only agency in metropolitan New York whose exclusive mission is to provide child welfare services for severely disabled and/or chronically ill children and their families. NAC's mission is to ensure that medically fragile children are not hospitalized beyond medical need, to open a world of opportunities to them, to give them a permanent home and family in the community and to support the whole family in reaching their potential. NAC's innovative services achieve permanency through timely adoptions or family reunifications. No children are allowed to languish in foster care and none has ever returned to live in a hospital. During the course of NAC's 18-year history, over 1,370 children have been served.

Annually NAC now reaches more than 350 children with disabilities and their siblings and families through one of four programs: Preventive Services, Foster Care & Adoption, PIP (Partners In Parenting, a prevention after-care program), and PLAN (Post Legal Adoption Network). What is the New Jersey Commission for the Blind and Visually Impaired?

The New Jersey Commission for the Blind and Visually Impaired is the designated state agency that provides services to New Jersey citizens who are blind and visually impaired.  Vocational Rehabilitation services are provided pursuant to Title IV of the Workforce Investment Act of 1998.  In addition to vocational rehabilitation services, the Commission provides services to eligible pre-school and school aged children through the educational services program and provides independent living services through the Adaptive Living Skills and Eye Health Services programs.  There is also a Prevention Services Unit in the Commission.  The Commission operates an Instructional Resource Center for individuals served through the educational services program and a comprehensive Rehabilitation Center for transition students and adults.

What is The New Jersey Developmental Disabilities Council?

The New Jersey Developmental Disabilities Council gives people with disabilities and their families a voice in public policies which affect them.  It funds projects to advance policies the Council supports.

The Council has several other efforts in the areas of housing, employment and community capacity building. The Council is comprised of 32 public and private members appointed by the governor under terms of State legislation and federal mandate. The Council is the only independent advocacy agency in the State dedicated to advancing the independence and productivity of people with developmental disabilities.

What is the New Jersey Department of Labor?

Offers several programs that benefit New Jersey's employers and their workers. Customized Training provides matching grants to employers to upgrade worker skills to enhance the creation and retention of high wage, high skill jobs. The Rapid Response Team assists employers that are restructuring and contemplating layoffs, by offering on-site services including reemployment, unemployment insurance and retraining services. The division offers free occupational analysis of workplaces to increase productivity and recruitment and training initiatives including the School-to-Careers Opportunities Initiative and Youth Transitions to Work Program.

What is the New Jersey Division of Disability Determination Services?

The New Jersey Division of Disability Determination Services processes and adjudicates claims for disability under Title II and Title XVI of the Social Security Act. Our mission is to render disability determinations decisions correctly, timely, and in a cost effective manner. To that end, we have four geographical claims regions to serve the State of New Jersey: North and Upper North in Newark, NJ, central in New Brunswick, NJ, and south in Trenton, NJ. The Division employs approximately 350 employees and makes decisions on about 80,000 claims per year.

What is the New Jersey Disability Determination Services?

The New Jersey Disability Determination Services (DDS) is a division of the department of Labor fully funded by the Federal Government, and is responsible for developing medical evidence and making the determination on whether residents of New Jersey are or are not disabled under Social Security disability law. While Social Security Administration handles the technical eligibility issues, a two-person adjudicative team consisting of a physician or psychologist and   disability adjudicator makes the determination on medical / vocational / legal eligibility. After DDS makes the determination, it returns the case to the Social Security Administration for appropriate next action depending on whether the claim was allowed or denied. If the claimant was found disabled, SSA prepares necessary payment actions. If the claim is denied, SSA retains the file in case the claimant wants an appeal.

What is the New Jersey Developmental Disabilities Council?

The Developmental Disabilities Council gives people with disabilities and their families a voice in public policies that affect them. It funds these projects to advance the policies it supports:

Election Challenge Grants, in collaboration with the Council's Monday Morning Project, have registered 8000 new voters with developmental disabilities and assisted many of them to get to the polls on Election Day over the last 6 years, thus increasing their participation in the activities of their communities.

The Office of Disability Services, created in response to the Monday Morning Platform, now has Division status and its budget for personal assistance increased ten-fold through Medicaid Funds.

Over the last 4 years, the Council has recognized personnel in 51 school districts with Exemplary Practice in Inclusive Education awards for developing innovative ways to support students with significant disabilities in the regular classroom. Winners receive a small stipend to defray the costs of sharing their expertise with other school districts throughout the state.

The People Support Project assists 344 people living in developmental centers who are moving to the community as a result of the Olmstead decision to prepare for their new lives. First of its kind in the nation, its provides training in individualized planning, exposure to community living options, group and individual supports and advocacy services.

What is the NJ Division of Disability Services?

The NJ Division of Disability Services is a part of the Department of Human Services. It offers a "Single point of entry" for individuals with disabilities (regardless of age or nature of disability) into the human services system.

The office provides comprehensive information & referral services, publishes a statewide directory of services and a quarterly newsletter for professionals in the disability field.

What is the NJ Division of Mental Health Services/Supported Employment Programs for Persons with Psychiatric Disabilities?

The NJ Division of Mental Health Services is the state mental health authority in New Jersey. Its mission is to promote opportunities for persons with serious mental illness to maximize their ability to live, learn, work and socialize in communities of their choice. This is accomplished through a comprehensive system of care including inpatient psychiatric setting and community based support services. The Division operates four regional and three specialty hospitals. It purchases community mental health services through contacts with about 125 not for profit providers. Community services include emergency screenings, outpatient partial care, supported employment, residential, integrated case management, and programs for assertive community treatment and systems advocacy.

What is New Jersey Protection and Advocacy, Inc.?

New Jersey Protection and Advocacy, Inc. (NJP&A) is New Jersey's federally funded Protection and Advocacy system for persons with disabilities.  NJP&A is a private non-profit organization established to advocate for and protect the legal, civil and human rights of citizens of New Jersey with disabilities. 

The New Jersey Client Assistance Program (CAP) is a federally funded program that advocates for and protects the rights of individuals with disabilities who are seeking or receiving vocational rehabilitation services.  CAP provides individual legal representation and non-legal individual and system advocacy.   CAP also provides outreach and education.  CAP usually works through negotiations, but is authorized to pursue legal, administrative and other appropriate remedies. 

CAP is an integral part of New Jersey Protection and Advocacy, Inc. (NJP&A).  CAP is funded by the U.S. Department of Education/Rehabilitation Service Administration.

What is New Jersey State Office on Disabilities?

The New Jersey State Office on Disability Services is located within the NJ Department of Human Services.  It was created in 1997 to serve as a central source of information and referral for people with disabilities, regardless of age or nature of disability.  The office has trained information and referral staff who assist New Jersey residents in location information, services and supports.  The office produces a resource directory and maintains a website.  In addition, the office administers the state-funded Personal Assistance Services Program (PASP) which provides personal assistant services to physically disabled adults, ages 18-65, who are employed, preparing for employment, or active volunteers in their communities.

What is Brooklyn Occupational Training Center (NYC Board of Education)?

This mission of the Brooklyn Occupational Training Center (BOTC) is to have young adults with disabilities play an active role in planning for their future by realizing their talents, strengths and capabilities. The program at the BOTC helps students make informed choices about their futures by facilitating their acquisition of specific academic and work skills, appropriate work habits and supports necessary to ensure community inclusion.

The Brooklyn Occupational Training Center is a New York Board of Education, District 75, special education secondary school for developmentally disabled young adults. The BOTC provides these IEP diploma bound students with a well balanced program that stresses in-school and community-based/vocational experiences. The BOTC utilizes an "immersion" approach founded on the premise that everyone can and should be allowed the opportunity to be a constructive and contributing citizen regardless of their disability. This system helps identify and put into place the community and the world of work.

The Brooklyn Occupational Training Center is committed to effecting a permanent, positive change in the lives of young men and women. Linkages between the school and the business community are maintained to support community based learning experiences for students. These experiences serve as an introduction to employment for our students and familiarize potential employers with the students.

In the last year of school, the student, their family, the school, VESID (The Office of Vocational and Educational Services for Individuals with Disabilities) and an adult service provider work together so that paid employment can be secured. A person centered or life plan (PCP) is developed by the team to help determine which employment opportunities are viable considerations. This is based on the student's past vocation/work experience, hopes, dreams and special accommodations or supports. The school and adult service agency jointly implement the goals of the PCP and are responsible for the development of employment opportunities, job coaching, and other supports the student may need in order to make a smooth and successful transition from school to work.

The Physically Handicapped Children's Program (PHCP) is a diagnostic and medical treatment and rehabilitation program administered by the NYC Department of Health, under guidelines developed by the NYS Department of Health.   The goal of the program is to ensure that physically handicapped and chronically ill children minimize the disability and maximize rehabilitative services so that they can reach their full potential.  PHCP provides a means of "last resort funding" limited to a maximum of $1500 per year of eligibility per child, and cannot substitute for or duplicate coverage by the family's private health insurance or any other third party payer.  PHCP also provides care coordination services through its Children with Special Health Care Needs resource program.  The purpose of this program is to help assure accessible, appropriate coordinated, culturally competent, comprehensive, community-based care for chronically ill and disabled children (ages 0-21 years of age) and their families.  These programs do not perform clinical services.
