INTRODUCTION In February 2006 we provided SSA with a preliminary version of this deliverable, a catalogue of transition resources. The contract calls for an update every six months. The February deliverable was limited to a list of national resources. During the last six months Westat worked with each of the 22 demonstration sites to develop a catalogue of community level resources. This August 2006 version of the deliverable includes both the national and community resources that will be provided to each beneficiary enrolled in the MHTS. However, each beneficiary will receive a manual of national and local resources that is customized to their local area. Soon after a site signed a site agreement, Westat began to work with the research assistant (RA) at the site to develop a community resources manual. The RA was sent a survey requesting a listing of community resources in each of ten community resources categories (provided in Chapter 3, Community Resource) or recommend an existing manual if it provided information for each of the ten categories. Westat reviewed each submission, and developed a resource manual for each demonstration site that contained both national and community level resources. A handful of sites received approval to use their own manual. For these sites Westat has produced a separate manual with just the national resources. As requested in the contract, control group beneficiaries will receive the resource manual immediately following randomization. Treatment group beneficiaries will receive the manual at transition, between months 18-24 of their 24-month intervention period. In Chapter 2 we present the national resources. Chapter 3 lists the 10 categories of community resources included in each community resource manual. Chapters 4 through 25 provide the current listing of community resources for each of the 22 demonstration sites. 2. NATIONAL RESOURCES This Chapter lists organizations across the nation that provide services to individuals with metal illness. Organizations are listed under seven different categories depending on the types of services provided. An organization that provides multiple services is listed under multiple categories. The survey lists 10 categories Westat felt would be valuable to beneficiaries in the control group looking for available services and those in the treatment group transitioning out of the study. For each organization listed in the manual, Westat requested the organization?s name, a brief description, address, telephone number, website address, and a contact person at the organization. Westat required the RA include, at a minimum, the organization?s name, description and telephone number and/or website. Upon completion, the RAs sent the surveys to Westat electronically. Sites that chose to provide beneficiaries with their own Community Resource Manual were asked to submit a hard copy to Westat for review. Following finalization of each manual, Westat will reproduce and send sites copies for distribution to each enrolled beneficiary. Community Resource Manual Status.
