How to Enroll

CBSV Enrollment Information and Application

SSA has reopened enrollment for its CBSV service. SSA is now accepting new applications from private businesses and government agencies to use CBSV. You must have an Employer Identification Number (EIN) to enroll.

Steps to Enroll

1. Complete the CBSV Enrollment Application and return it to us by email to ssa.cbsv@ssa.gov.



2. Enclose your check payable to the Social Security Administration in the amount of $5,000, or complete a Credit Card Payment Form, and mail it with the completed CBSV Enrollment Application to:

3. Upon receipt of your enrollment application and the $5,000 enrollment fee, SSA will initiate the formal agreement process by which SSA and you will execute a User Agreement memorializing your participation in the CBSV process.

SSA will not refund the $5,000 enrollment fee. Your submission of the CBSV application form, along with the enrollment fee, constitutes your acknowledgement and agreement that the enrollment fee is non-refundable.
